Special Notes
Written correspondence or verbal communication requested by the client — including reports, referral forms, progress or doctor’s notes, phone calls, etc. — as well as any therapeutic materials provided (e.g., low-tech communication boards), will be billed at a rate of $150/hour, $130/45min, $100/30min, $50/15min.
A chart review fee may apply in certain situations, (e.g. transferring to a different therapist’s caseload, lapsed care or extensive document review). This will be billed at a rate of $150/hour, $130/45min, $100/30min, $50/15min.

Oscar’s Place is a private rehabilitation centre and, as a result, services are not covered by OHIP.
Services must be paid for at each session. We will happily directly bill most commonly known insurance plans. A receipt will be provided once payment is made in full and this receipt can then be submitted to another funder (outside of directly billed extended benefits) for reimbursement.
We require a credit card on file to schedule the first appointment or 50% of the session cost.

The client chart can be shared to the caregiver by making it “viewable” by the treating therapist through the secure Jane app. With consent, a pdf copy can be emailed to the primary email address on file.
Photocopy and Computer Print Out Fee. You will be subject to a $0.25 fee per page after the first 10 pages.
Cancellation Policy
Our goal at Oscar’s Place is to provide exceptional care to our clients and their families. We strive to provide care in ways that best meet the needs of your child as well as accommodate your family’s schedule. However, missed clinic appointments and frequent cancellations do impact our ability to provide quality services. If your child is unable to make their scheduled appointment time due to illness, we ask that you provide the clinic with as much notice as possible, 48 hours notice would be appreciated!
First Missed Appointment (without notice) or Cancelled Appointment (within 48 hours of appointment time) will be subject to 50% of session fee.
Subsequent Missed Appointments or Cancelled Appointments (within 48 hours of appointment time) will be subject to 100% of the session fee.
How Do We Share This Policy?
1. This policy is shared on our website under the “Service Fee Schedule and Cancellation Policy” menu tab.
2. It is shared on all new client intake paperwork. This requires the client (or their legal guardian) to sign off that they have reviewed this policy.
3. It is posted on our administrative desk.
Collecting Payment
Payment for services is collected on the date of service and a receipt is provided. If the client is unable to make a payment on the date of service, a payment plan can be created to support the client’s ability to pay for services.
If the client has failed to pay for services (or failed to comply with payment plan) for 2 weeks (14 days) since the date of service or creation of the payment plan, an additional attempt will be made to receive payment.
If no receipt of payment within 30 days of date of service or establishment of the payment plan, client discharge may be considered.
Late Client Policy
If you happen to be late for an appointment, we will make every effort to see you for the remainder of the scheduled time. The full fee will be charged for this appointment. Thank you for your cooperation!
